Taken directly from the DIY templates, I have modified the contact pages into forms that will allow you to type your contact information and then save the document for future editing.

Although I have made this template a form that can be edited, I have only modified a DIY template and give them all the credit for the actual document. This is just something I have always wanted, but never been able to find. Please let me know if you find anything that needs modified.

Depending on how you use this form, I suggest entering your information, then saving the document. (IE: Contacts, A; Contacts, B; etc.)

Doug, I hope you don't mind me doing this. If you do, please just delete it.

2007 One Month per Page

I needed a one month per page layout... but ran into some problems with printing since I wanted them duplex.

I'm sharing the solution here :o)

I used the marvelous free pdf calendars available here : SCIRAL .

I used Illustrator to reformat them and put them into the proper order. I allowed room for punching and the .25" margin that most printers require.

Simply print duplex, slice down the middle, punch and utilize.

Migraine Headache Journal

If you suffer from headaches, the most useful thing for your doctor to help you diagnose it is a journal of the most recent instances. This form design was inspired by a pamphlet at a doctor's office, only this one gives you way more room to record things properly.

The form has columns to record the date, start & end time, the possible triggers, the symptoms and the treatment you tried. At the end of each page is a reminder of commons trigger sources for migraine type headaches. There is room for a lucky 13 headaches per page.

This is a classic sized page, with two pages on a single 8.5 x 11 (print and cut in half for two pages). This was created using Microsoft Excel, and printed to an Adobe PDF format.

Disclaimer: I'm not a doctor, just a lifelong headache sufferer. Use at your own risk. Take two of these and call someone else in the morning.

Usage advice: 

When the headache first begins, record the time. Record what it feels like (pain in the forehead, back, all over, etc.). Look at the possible triggers list and try and remember exposure to any of those items in the past 24 hours.

In the treatment column, record any pills or other techniques you used to try and alleviate the pain. When the headache ends, write down the time.

Steno pad paper classic 2up

My husband (whom I hacked) requested that I make him a notebook in classic size with steno pad-type paper. I fiddled and fiddled with both MSWord and OOo and here is the result. He wanted it so there were no borders on the sides.

Dive Log

I needed new pages for my dive log and rather than buy some overpriced pages printed on paper that is nearly impossible to write on, I decided to make my own. The log has space for a dive profile, some summary information, comments, your cumulative bottom time, and a verification signature.

Print, punch and use just like your normal dive log. This template only has a simple single level dive profile on it, I may get around to creating one with a multi-level profile one of these days but since I don't have the equipment to do those calculations it's not super high on my to-do list.

WW Flex Point Tracker - Seven Days Across Two Facing Pages (Classic)

This template allows the user to track their daily points as well as daily water, oil, milk and fruit and vegetable intake. There is also a place for tracking weekly flex points and activity points.

The thing I like about this layout is that all seven days of the week are in view at one time with Days 1 through 4 on the left page and Days 5 through 7 on the right.

The downside is that the available space for notations is small as compared with WW's own tracker. The upside is that it fits right in my planner.

Character Classic 2-Up

This was something I needed.
I took the Character form from the Hipster PNG Image set, and put it into a MS Word file, Made it bigger, and put 2 of them. I made it in about 10 minutes, so it isn't that great, but I needed it (my first attempt at making a form and my first upload, by the way).

Fiore Unschedule

A blank 24 hour, 7 day template on a single classic page. Used as an anti-procrastination tool in Dr. Niel Fiore's The Now Habit.

Created using the Widget Kit in NeoOffice Draw (the Mac OpenOffice port) on my Macbook.

Note: My day rarely begins before 4am, but will occasionally end there, so I set this template up to reflect that. You can play with the Ooo files to get a more conventional midnight to midnight (or whatever you want) span.

All rights to those who hold them. I was just monkeying around. :)

(This was my first time using the Widget Kit, open source software, or a drawing program, so please bear with me. I'm still not sure about the quality of the PDFs.)

See Fiore's The Now Habit for detailed usage. Highly recommended. I've also been to DIY Planner member flexiblefine's yahoo group on the topic, and recommend that as well.

The basics: 1) block out all hard landscape commitments, 2) schedule and block out "guilt-free play" and exercise (stick to it!), 3) track projects or activities you're procrastinating on by a) not scheduling work on that project to the hard landscape, b) "limiting" yourself to "no more" than 20 hours a week on that project, and c) tracking work done on the project in the open spaces of your Unschedule, recording only when you've done a minimum of 30 minutes uninterrupted work. Use the boxes at the bottom of the page to tally the total number of hours you've recorded for the project(s) you're focusing on.

Medical Data

When my mother was diagnosed with Cancer we recieved several different forms that were designed to put all of her medical data in one place.

I have adjusted the forms that we recieved and added some things of my own. This will print out on a single 8.5 by 11 sheet of paper and can then be folded over and holes punched in it for your classic organier.

When I was looking through the forms I felt that this would be good for me to keep my information and others might be interested in this handy way to keep their medical data up to date.

The form is fairly self explanitory. Once the information is filled in fold it and use your punch to put this in your dayplanner for quick reference or put it on the refrigerator door as one of the groups told us to do when we recieved the forms.

2 Page per Week Planner w/Task List

I've developed this template, fully merged for 2008, based on my 3 years of working with the Harvard Planner.

I've taken some of the useful (to me) elements, and rearranged them. First, I've rearranged the weekly calendar across the top half of the two pages, Mon-Wed on the left, Tr-Sun on the right. The task, which were on the right page of a Harvard, are now under both pages. Gone are all the small columns for weekdays and checkoffs. There's a simple single column adaptable. If it's a calendar event, put the M,T,W,TR,F, etc in the box; if it's a task, I use a GTD-esque dash system there.

All new! I've added the first 24 pages of my now complete year. The new setup includes the following, front to back, with thanks to ygor:

DIY blank cover
DIY Personal Info sheet
Blank spacer page
2008-2009 2 page per year calendars (thanks ygor)
January 2 page per month overview calendar (thanks again, ygor)
January pages, described above.
Repeat for each month.
Ends with several note pages as spacers.

Take a look. Let me know what you think. I'll look for another location for the full file - it's too big to post here. Most likely caused by merging several pdfs with pdfsam.

Special Order 2 page/week - classic 2up

The result of a member's request for a custom 2-page per week form, to fit her Circa Junior Zip Folio. Left-hand page has Monday through Friday; Saturday and Sunday are on right - all days equal size. Rest of right-hand page has grid split into two columns with "To Do" and "To Buy" headings.
Please let me know if there are any major tweaks that need to be done.

24/7 Work/Life Planner

This is two-page per day classic size template based on the 24/7 Work-Life Planner featured in the new time management book: "Attack Your Day" by Mark Woods (and his late Father Trapper). The date is left blank so you can print as many as you want and add the dates to them.

This works well on both seven ring planners and Circa punched junior books also. Your preference.

Daily Mood Journaling / Tracking Page

This template is primarily aimed at those suffering from Bipolar disorder or depression. It is meant to keep track of not only your moods, but also the events and variables surrounding them so that you can target things that trigger your moods so that you may balance them.

I really, really hope this can help some folks out. If anyone has any suggestions or questions, please, let me know. I'd be happy to help in any way I can. Also, this is my first template, so go easy on the negative comments, please.

You could probably come up with several systems to use with the template, but what I had in mind with it, was this:

During the day, mark a dot on on the "Mood Tracking" grid, showing where your mood was at during what part of the day. (Hint: There are 24 blocks going across!)

Also, keep track of how many hours of sleep you had the night before, what medications you took, what you ate, how long you exercised, and the weather.

At the end of the day, write a short blurb under daily blurb, describing how you felt throughout the day, or any event that stuck out in your mind. Also, note what your "overall" mood was during the day.

At the end of the week, or month, whichever you prefer, go through your mood logs and try to see if there are any patterns in diet, exercise (or lack-there-of), weather, sleeping habits, etc. that may be contributing to your depressed or manic bouts. See if you can find a common factor in the times you felt "normal" and try to repeat those things.
