Template: sketch at the top, lined at the bottom (or left and right)

I like to sketch designs and then write a few notes on the materials I need for the design, instructions, etc. I don't know how to create a template or I would do it myself. Can someone either tell me how to get a basic template and make what I need or create it for me?

Sounds like...
you need a storyboard template.
Try the Classic D*I*Y Planner 3.0 Creativity Package, pages 9 thru 20 or so for some various styoryboards pages.

To Roll Your Own ...
... First, download OpenOffice.org and become familiar with the Draw section. Expertise isn't needed.
Then, download the DIY Planner Widget Kit. It is a Draw document with all the "parts" to build customized pages in the DIY Planner style.
1. Make a copy of the Widget Kit file with a new name
2. Open the new file, insert a new (blank) page, copy and paste the right widgets to your new page. Save early, save often!
3. Tweak as needed, add text as desired. Save some more.
4. Finally, delete the unnecessary pages from your file. Print.
