Help...DIY Newbie...Out of my element

Hello All! I'm a pharmaceutical rep with a very specific idea of what I want in a planner, however I'm ill equipped to create my own template and wondered if someone else was up to the task?

Hi I am looking for a very open format. 1 pg/day with 4 sections: Appointments, Lunch (to include a line for Locale, Time, # of attendees), Plan and Notes. Something that looks a little like this:

I'd like the dates to be pre-populated. Some interesting but plainly legible font with a nice, clean crisp color scheme. The size to be 5 1/2 by 8 1/2. My handwriting is about medium in size so I don't need too much room between lines, the typical size available in most store bought planners works fine for me.

Thank you so much for replying to my request. I know what I want I just lack the capabilies to create it for myself. I'm a pharmaceutical rep and I don't have a lot of appointments, just "best times" to call on my customers, so I've never needed those planners based on appointment times. I just need a small place to keep the few appointments I do make, space to plan my day with a list of physicians I need to see, and have space to take notes for follow up or business tracking. I do the majority of my business over lunch so I have those 3-4 times a week.
