We like to keep a record of the books read for personal enjoyment during "Reading" time. With extra large spacing, even the youngest writer should be able to copy the book title into the form. Stars for a rating of the book read may be colored in, if desired.

Usage advice:
Book title on the left then date followed by a star rating. The date may either be a range for longer books or the date read for shorter ones. Your preference, of course.
