Looking for Phone & Address Directory Templates or Designs

Looking for a Phone & Address Directory template that is flexible to add further emails, addresses, kids & pet names, etc. I currently have all my contacts details in excel and love the DIY Planner style pages. I have a Filofax personal size or A5 would suit. Any templates (Excel, word (mail merge) or ideas would be much appreciated.

Thanks to you all in advance and for all you wonderful ideas, it just keeps me enthused and glued to this site.

Contacts

Hi.

I decided not to use a template for my contacts. I find they waste a lot of space for most entries, and don't have enough for some.

So I divided my contacts into categories--Work, Medical, Household, Family. The first three lists are one page, with just a list of name and phone number(s). The family list is one page per family, basically, and includes addresses, birthdays, anniversaries, kids' names, pets' names, work and home info, etc.

I keep them in a simple OpenOffice Writer document and reprint them occasionally when I have a critical mass of handwritten updates. :)

Printed copy with handwritten updates and periodic re-printing.

Here is my Excel headers -- similiar to what has been discussed

These are the headers I use for my address pages -- I use at least two lines per "contact" in my friends & family section... if married I will insert the spouse information (and in some cases children's information too) between the Last name row and address row.... especially since cell phones (and birthdates) are distinct. When there is an addition, I insert the appropriate number of rows, and key in the data -- on my printout I always keep about four-six sets of lines for additions:

Last Name First Name Home Phone Office Phone Cell Birthdate Anniversay
Address City State Zip

In a separate worksheet in the same workbook I have a tab for business contacts. The headers I use for business contacts is simpler -- one line. If you are using a smaller page format it could be made into two lines. I find this area to be more active than my freinds & family portion -- so have tried to keep it to a single line so I can sort.

Again I always print out a number of additional lines so I can add data as new business contacts come into my life.

I like simplicity. It is hard to have a manual system that allows alphabetical insertation of future contacts. At home I just use a Moleskin A-Z notebook and leave lots of space... still wind up with conflicts over time.

Detailed Contacts Template

I have tweaked the contact template from the basic template pack to include multiple e-mail and phone lines plus a family / children section with a space for birthdays and other. It's 3x5 pdf but it should be easy enough to resize if you give me the dimensions you want.

I still use the original contacts template for work / dr / dentist contacts and use one page per person of my detailed contacts for freinds and family.
