My gerneral rule-of-stuff is, "handle it only once if at all possible," so the only things that are supposed to end up there are future things that can't be dealt with today.

As do I. When I get interrupted, I rip off the top and move it up to a large side opening Rubbermade wiremesh inbox. When I have the inbox empty, I'm done (or I need to add more items).
All my context lists sit on 3x5 cards spread out over my desk to the far right.

I use an Avery 75254 in the front of my binder as an inbox, and at the back of my binder to transport stuff home. I have a few more inboxes, though:

How about a plain old legal pad or notebook (such as a Moleskine?)

I have a 3x5 pad at the back
I have a 3x5 pad at the back of my planner that I jot things down on. At the end of the day or when the sheet is full, I process them.
