Cornell index card with task doodads

I really like the cornell notetaking system, but I prefer to use it on index cards, with a column for tags and a little section at the bottom for followup tasks. So I made a template. Yay!
Give it a go and let me know what you all think.

Usage advice:
Fill in the title at the top (I use the format lecture [card no.], with most lectures spanning multiple cards).
Note any followup tasks that occur to you during the lecture at the bottom (I guess you could use a separate tasks card for this, but I like to keep them together so I process the tasks at the review stage).
Later (say, that evening or a few days after the lecture), take your notes and read them through, tagging the card appropriately.

Four Up
It would be cool if you could set it up four on a page. My printer doesn't print on notecards very well at all. I keep a ream of cardstock handy to print out the other DIY hipster forms. It would be nice to add this form to the list.

mystery second page
Every time I print this card I get a second page (on regular 8.5x11 paper) with a message on the top saying "PCL XL error Warning: IllegalMediaSource". I can just toss the second page each time but it is a waste of paper and a bit of an annoyance. Anybody else have this problem with this or other cards? Any ideas what might be doing it?
easy fix
I'm not sure what would cause this but here's an easy fix... When you print, just make sure you tell the program to only print page 1.

Printing only one page doesn't help. It still adds the 2nd page telling us "PCL XL error, Warning: IllegalMediaSource".
This is a new printer for our office. I am guessing that it is a setting that can be turned off, but searching the Internet I can't find the solution. Did the original questioner find the solution?
Hate to have to call the printer ppl.
